I agree to represent myself as a mentor, once the GSPCA BOD has approved me as a mentor.

I have read and agree to adhere to the GSPCA Approved Mentor Guidelines.

If I am no longer able to fulfill my duties as an approved mentor, I will notify the GSPCA J. E. coordinator to be removed.

I understand that if I exhibit conduct unbecoming of a mentor as outlined in the Guidelines, the GSPCA BOD may elect to remove me from the Approved Mentor list.
